This form should be submitted with applications for the Nonprofit Strategic Restructuring Initiative only.  Other applicants should not complete this form.   All organizations that are part of a planned strategic restructuring project must complete this form and submit it to the Dyson Foundation.  Ideally, this form should be completed and returned to the lead agency applying for funding, who will attach it to their application.  Alternately, this form may be completed by the partner organizations and emailed to This e-mail address is being protected from spambots.
